To best allocate time to every patient, we request that you inform us of any cancellation of your appointment at least 48 hours in advance. Please help us to better serve you and other patients in our practice by making every effort to keep your appointment and by informing us as early as possible of your cancellation or postponement.

There is a $50 new patient consultation fee, which will be applied to any surgical fees and is due at the time of your first visit. Second consultations prior to surgical procedures incur no additional charges. A non-refundable 15% scheduling fee is required to book your surgery date. This fee is applied to the total surgical fee. The balance is due at your pre-operative appointment which is scheduled two weeks before your surgery date.

We accept checks, cash, Visa, or Mastercard. We also offer financing through Health One Financial (see right column). You will also be financially responsible for any pre-operative reports, labs, prescription medications, and compression garments.
